DOPPS researchers track dialysis access placement patterns - NephrologyNews.com PDF Print

In a special webinar being presented today, researchers from the Dialysis Outcomes and Practice Patterns Study (DOPPS) will examine data from its Practice Monitor (DPM) on the state of access placement in the United States.

The Monitor is a sampling of over 200 dialysis clinics and 14,000 patients that generates reports with more than 1,500 regularly updated charts, figures, and data tables. The latest data will cover emerging trends in US hemodialysis (HD) practice through December 2014, with a focus on vascular access practices and international comparisons.

The DPM now includes over four years of trends in U.S. dialysis care, including comparisons by race, rural and geographic location, affiliated dialysis organization size, facility profit status, and hospital-based setting.

Key messages about access

Researchers will explain the practice patterns they see from data gathered by the monitor, including:

Though fistula use in the U.S. has risen among prevalent dialysis patients, the majority of patients still start dialysis with a catheter –– even those who see a nephrologist prior to starting  therapy.

Vascular access use varies widely across DOPPS countries. In Japan and Russia, 91-92% of hemodialysis patients use a fistula, but only 49% of Canadian patients do (in the U.S., it is at 68%) in prevalent cross-sections of hemodialysis patients.

Arteriovenous graft use was twice as high in black patients (26%) than others in the U.S.

Despite large improvements in arteriovenous fistula use in prevalent hemodialysis patients during the last 15 years, arteriovenous fistula use has remained low among new patients when initiating HD and has shown essentially no improvement in 15 years.

Renal transplant ICU to reopen in a fortnight - The Hindu PDF Print

The renal transplant ICU at the Super Speciality Block (SSB) in the Government Medical College Hospital (MCH), which has been shut for over a month for maintenance of the air-conditioning system, is expected to resume functioning in two weeks’ time, MCH authorities have said.

Renal patients awaiting transplants, after completing all pre-surgical formalities, have had to go back to dialysis.

The repairs turned out to be complicated. “The coil of the air handling unit of the A/C needed replacement. The representatives of the A/C firm said that it was a custom-made spare, not readily available. Orders had to be placed with the company. They said the standard time for manufacture, delivery and installation was six weeks. We have issued the supply orders and are awaiting the spares,” sources said.

Though there is an annual maintenance contract for the A/C systems, expensive spares – this one costs nearly Rs.3 lakh – are not covered under the contract.

The alternative was to vacate another ICU or have another area converted as ICU, but this was also not possible as other maintenance works are also on in the hospital. “Organ transplant is risky and unless the patient is maintained in a sterile environment, there are possibilities of post-surgical complications or even death,” a transplant surgeon said.

Renal transplant successful - The Hindu PDF Print

Global Hospitals on Wednesday claimed to have succes-sfully performed incompatible renal transplant on a 24-year-old suffering from chronic kidney failure. The patient had not gotten a suitable blood group matched donor, follo-wing which the hospital took up incompatible renal transp-lantation using plasma-pheresis and Rituximab, said G. Sridhar, chief consultant nephrologist of the hospital.

Renal transplant is rejected in cases where blood groups of patient and the donor do not match. However, recent advances have made succe-ssful transpl-antation possible even without blood group matching, Dr. Sridhar said.
